Signup forms

Create embedded and popup forms, publish with embed code or a hosted link, and track conversions.

Use Forms to grow your list with branded signup forms you can embed on your website or share as a hosted link.

Create a form

  1. Open Forms in the main navigation.
  2. Click Create form.
  3. Choose a name, target list, type (Embedded or Popup), and template (Inline, Card, or Horizontal).
  4. Save, then customize copy, colors, fields, and consent on the form editor.

Set Status to Active before publishing. Draft forms are not publicly available.

Fields and consent

  • Email is always required.
  • Optionally collect first name, last name, and custom fields (comma-separated keys).
  • Enable a GDPR consent checkbox with your own wording; mark it required when needed.

Double opt-in still follows the target list’s onboarding settings.

Publish

Hosted form URL

Every form has a Mailivate-hosted page. Share it on social bios, QR codes, or anywhere you do not control the HTML.

Embed code

Copy the snippet from the form editor and paste it on your site:

  • Embedded — renders inline where you place the mount div.
  • Popup — loads the script and shows according to your trigger settings.

Popup triggers

For popup forms, choose:

  • Delay — show after N seconds
  • Scroll % — show after the visitor scrolls a percentage of the page
  • Exit intent — show when the cursor leaves toward the top of the page
  • Manual click — show when a CSS selector is clicked (for example #open-signup)

Also configure:

  • Show once every N days — frequency capping via cookie
  • Hide if already subscribed — skips the popup for returning subscribers

Stats

Each form shows:

  • Views — times the form or popup was displayed
  • Submissions — successful subscribe attempts counted toward conversion
  • Conversion — submissions ÷ views

List HTML forms (advanced)

Lists still support a raw HTML POST form under List → Onboarding. Prefer the Forms product for branding, popups, hosted links, and stats. See Onboarding and subscribe forms.
